Ascend provides telehealth services, including behavioral health care, counseling, and related support, through licensed healthcare providers ("Providers").
Our services are not for emergencies. If you are experiencing a medical or psychiatric emergency, call 911 or go to the nearest emergency department.
3. No Medical Advice Guarantee
While our Providers deliver medical and behavioral health care, Ascend itself does not practice medicine. All medical and clinical decisions are made by independent Providers based on their professional judgement.
4. Telehealth Consent
By using our Services, you acknowledge and consent to receive healthcare via telehealth, which may involve:
● Video, audio, and digital communications
● Secure transmission of health information
● Risks related to electronic transmission failures or security breaches

5. User Responsibilities
You agree to:

Payment terms, including insurance billing, co-pays, and out-of-pocket costs, will be disclosed prior to treatment. By receiving services, you agree to pay all applicable fees and charges.

These Terms are governed by the laws of the State of Arizona, without regard to conflict-of-law principles. Any disputes will be resolved exclusively in the state or federal courts located in Maricopa County, Arizona.
